| Description | Forms collected by the Registrar and completed by students enrolled for Masters and PhD degrees in the Faculty of Science at the University of Birmingham containing name, date of birth, place of birth, nationality, permanent and sometimes local address, date of entry to University, name of school, particulars of examinations, name, occupation and address of parent or guardian, brief details of course of study, and printed declaration promising to obey the Regulations of the University signed by student. There are some forms dating from the early 1940s, and a complete sequence of forms for students registering for higher degrees in the Faculty of Science between 1948 and 1954.
